  1. Top boom wear pad missing, keeper plate still in place, note metal filings
     
  2. Plate work on 150 ton Lima inner boom (bottom plate) 2 cracks
     
  3. This lattice broke when crane (American 7530) shock loaded the boom while Rolling a press that weighed 60 ton±
     
  4. This boom hoist wire rope was still on the crane when I found it, crane being used every day, in fact they had a sister crane that lost the boom for this reason several weeks prior. The Operator did not know how to do a daily inspection nor know what to look for.
     
  5 These gears were found on a 3900T, brass is flaking and was building up on the worm (boom hoist gear case)
     
  6 Note the wear and pin holes in this Link-Belt boom hoist gear case
     
  7 This is a broken swinger gear case I upper bearing housing on a Galion RT
     
  8 This boom was on a Clark Lime 990, The operator (new to the crane) did not know Where to put the belly band on the off-set jib and boom section, so he wrapped the Wire rope around the lattice and cords, then when he installed the back-stay lines he put them on the wrong ears on the boom and did not have a straight pull on them Note the bend in both lines on the boom.
     
  9 This Grove TMS 250 had the main frame cracked at the rear drivers, 18” vertical, The bearing bolts were loose and carrier frame had many cracks, crane had to be Torn down complete and repaired
     
  10 Hurry up weld on lattice of the Lima 990 jib section
     
  11 This truck crane had all its outrigger boxes split in the center weld areas bottom side
     
  12 These foor wedge sockets were found in the same mine, on different cranes 12a has An oversize wedge for the size socket I wire rope,
     
  12b We always use a fine thread machine bolt for the dead end tail,
     
  12c This socket was reeved backwards but looked OK after they two blocked the load block, then the rope hung straight., also they used a fist grip to clip the two ropes together, wedge also was wrong size undersize).
     
  12d Wrong dead end pin for socket, (cold rolled stock about 8” long undersize, cotter pin about to pull through socket ear)
     
  13 The rope on this winch was run off and wrapped backwards on drum, operator Noticed block going up instead of down, reversed winch and forgot about it, Hotice Rope is crushed and kinked, missed the lagging one wrap and would not wrap Straight on drum through all layers
     
  14 This is not a factory weld job repair
     
  15 Steering cylinder rod backed out of clevis end on a P&H 18 ton RT, they were still using crane until I tagged it out
     
  16 My friend looking over his crane after an accident, he was told a wrong weight Of the piece he was lifting and told to disregard the load cell in the crane. Foreman was fired for this. He was reading the wrong blueprint for the weight of the turbine shell they were lifting. My friend never got back in a crane after this. Nobody got hurt either in the accident. This sure would make looking at the carrier undercarriage easy wouldn’t it?
